Transitions LifeCare is a nonprofit providing expert hospice, palliative, and serious illness care to patients and families across central and eastern North Carolina. We believe every individual deserves the quality of life they want when facing a serious illness or near life’s end. Wherever you or a loved one may be on a path of serious illness or grief, Transitions LifeCare surrounds you with a comprehensive circle of expert care and support. We provide guidance, resources, and expert services to Wake, Chatham, Orange, Durham, Franklin, Harnett, Johnston, Granville, Wilson and Nash counties.
