About Us

CommWell Health is a not-for-profit 501(c)(3) Federally Qualified Health Center (FQHC) first established in 1977 to provide culturally and linguistically appropriate primary medical care to agricultural workers living in Johnston, Sampson and Harnett counties of south-central North Carolina. The vision for CommWell Health began as a partnership between three county health departments to address the significant gaps in care for agricultural workers. The vision took form as a single provider medical night clinic in a country store and has grown significantly in size, scope, and breadth into a system of primary care practice locations offering patient centered medical, dental and behavioral health and support services to all populations and communities living in six counties of southeastern North Carolina. Today, CommWell Health continues to provide the services that originally motivated its establishment along with new programs and services focusing on holistic, patient-centered community health care. CommWell Health is accredited by The Joint Commission in both ambulatory and behavioral health care and is certified as a patient-centered medical home.

As a Federally Qualified Community Health Center (FQHC), CommWell Health offers services to meet the health care needs of the communities served. CommWell Health serves everyone, regardless of their insurance status. Each CommWell Health site offers affordable, comprehensive care in a patient centered health home setting.
